Onboard Outreach provides community-based supports, support brokerage, and transportation services designed to promote independence, dignity, and meaningful access for individuals with disabilities. We work alongside individuals, families, and support teams to navigate services, build life skills, and remove barriers that limit participation in everyday life.

Our community-based supports are delivered by trained Direct Support Professionals (DSPs) who provide hands-on assistance with daily living, employment readiness, social engagement, and community participation. Through support brokerage, we help individuals understand their benefits, coordinate services, manage budgets, and make informed decisions that align with their goals and preferences. Our transportation services ensure safe, reliable access to medical appointments, employment, programs, and community activities, while also offering coordination and coaching to support long-term mobility independence.

At the core of our approach is person-centered planning. We believe support should be flexible, respectful, and guided by the individual’s voice. By combining practical assistance with advocacy and education, Onboard Outreach helps people build stronger connections, increase self-determination, and live fuller, more independent lives within their communities.
