Join us for a special conversation with NJ Commissioner of Human Services, Dr. Stephen Cha & NJ Deputy Commissioner of Health, Nashon Hornsby.
The conversation will focus on priorities, initiatives, and opportunities impacting families. Participants will have the opportunity to engage in dialogue around current challenges, emerging needs, and strategies to strengthen supports for children, youth, and families statewide.
The NJ Community of Care Consortium (CoCC) brings together state agencies, family leaders, healthcare providers, advocacy and disability organizations, youth, parents, and professionals committed to improving systems of care. The Consortium works to address disparities in health outcomes and strengthen family-centered systems of care across New Jersey through collaboration, advocacy, and partnership.
CoCC Workgroups: Each workgroup is co-chaired by a parent leader and a professional partner:
- Infant Health / Early & Continuous Screening
- Child Health / Medical Home & Community Resources
- Adolescent Health / Transition to Adulthood
- Maternal Health / Adequate Financing
