NJ Social Services (NJSS) is a trusted and mission-driven Support Coordination Agency proudly serving adults aged 21 and older with developmental disabilities throughout New Jersey. Operating under the guidelines of the New Jersey Division of Developmental Disabilities (DDD), NJSS is committed to delivering person-centered, family-focused support that enhances the lives of those we serve.

Our agency specializes in Support Coordination Services for individuals enrolled in the Supports Program and Community Care Program (CCP) through DDD. These services are essential in helping adults and their families navigate the often-complex system of Medicaid-funded programs, community resources, and provider networks.

At the core of NJSS is a team of dedicated and compassionate Support Coordinators who work directly with individuals and their families to develop and maintain Individualized Service Plans (ISP). Each plan is carefully tailored to reflect the person’s goals, strengths, and support needs, ensuring access to services that promote independence, safety, health, and community integration.

Our responsibilities include:

  • Conducting home visits and ongoing check-ins.
  • Connecting individuals to qualified providers of services such as day programs, employment supports, respite, and transportation.
  • Monitoring service delivery and ensuring quality and satisfaction.
  • Assisting with annual reassessments and plan updates.
  • Advocating for the individual’s voice to be heard and respected in all decisions.

Eligibility To receive Support Coordination services through NJ Social Services (NJSS), individuals must meet the following eligibility criteria established by the New Jersey Division of Developmental Disabilities (DDD): 1. Age Requirement: Must be 21 years of age or older. 2. Residency: Must be a legal resident of New Jersey. 3. Disability Qualification: Must have a developmental disability that began before age 22 and is expected to continue indefinitely, resulting in substantial functional limitations in at least three or more of the following areas: • Self-care • Learning • Mobility • Communication • Self-direction • Capacity for independent living • Economic self-sufficiency 4. Medicaid Eligibility: Must be eligible for and enrolled in New Jersey Medicaid. 5. Enrollment in DDD Services: Must be determined eligible by DDD and enrolled in one of its Medicaid waiver programs: • Supports Program • Community Care Program (CCP) 6. Tier Assignment: Must complete the NJ Comprehensive Assessment Tool (NJ CAT) to determine level of support needs, which will influence budget and service planning.
